Incident Report Form

The intended use of this form is to report or clarify an incident or problem
that happened during a Makule game. We ask that only those who were
directly involved, or had a direct view of the incident, submit a report.
When done, click on the "Send Email" button, and the report will be sent
directly to the Czar for review. You will be contacted in a prompt manner,
so a phone number and/or email address is critical.
