Registration deadline: mailed in by Monday Oct. 11, 2010 @5pm .
Late registration fee: $70 no exceptions!
All new applications for participation in the Makule Soccer League must include a 1 time photocopy of either a Driver's License, Birth Certificate or Passport, which shows the applicant's date of birth. If you are new to Makule Soccer you also need to fill out the "US Club Soccer Registration Form". This is needed for us to register the player with US Club Soccer, will be a one time requirement, and not needed for registration in successive seasons with the League.

To complete this registration form, the default is set on the first box; fill in the information, and use your tab key or mouse to move to the other fields. When done click "print this page" and remember to sign the printed out form before sunmitting. You can also print out the familiar pdf registration form from the Makule website, and fill it out by hand. When competed, mail in the form with your registration fee to the address below.
